為了不要造成每天追梗補梗的行為，

我決定勤勞一點快速的寫下它。


今天的遇到的依然是一位老客戶。
他叫做ganciclovir。


這東西真是出現率太高了，陪我玩耍了一陣子。


先前的文章中有介紹到ganciclovir會造成bone marrow suppression，

而今天就是因為神秘人A的WBC和PLT再次的下降，令人相當困擾阿
目前治療神秘人A的CMV，
使用ganciclovir仍處於induction therapy的第十天，
還差四天的induction dose。

但由於昨日測的WBC已掉到1700,PLT為48,000
感染科老大就先拉了煞車，選擇停藥
今天測WBC稍微回升到1900,PLT反而還更低變成41,000

這讓大家傷透腦筋阿。

(主任：再給我四天就好了阿~阿~~阿~~~阿阿！！！)


由於guideline上的CMV治療，不建議在induction時期停藥，
因此要如何減輕或治療thrombocytopenia和leukopenia就變成一大難題。

目前查了文獻只知道這些side-effect可能是dose-related性質，
但是相關的劑量減低和調整，筆者實在是找不到…


主任提出了以folic acid來進行thrombocytopenia治療，
但也不知道是否有效就是了…

至於neutropenia的部份則是決定當ANC下降到1000時，
施打GCSF。


另外補充一點，由RxList找到的資料

「Neutropenia typically occurs during the first or second week of induction therapy and prior to administration of a total cumulative dose of 200 mg/kg of ganciclovir i.v. but may occur at any time during treatment. Evidence of recovery of cell counts usually occurs within 3 to 7 days after discontinuing the drug. Colony stimulating factors have been shown to increase neutrophil and white blood cell counts in patients receiving ganciclovir i.v. solution for treatment of CMV retinitis」

Neutropenia可能會於 induction therapy 開始的一到二周內發生。
證據顯示停藥後3~7天內會WBC恢復。

根據Handbook of Applied Therapeutics,
當ganciclovir使用濃度高於10mg/kg/day或
患者有AIDS情狀時，neutropenia的發生率會更高。
